CONSENT ORDER

On November 14, 1995, this court ordered that Appellant complete "the single 23 day suspension by January 31, 1996." Due to the surgery of a close family member, appellant has requested and Respondent has agreed, upon condition, that Appellant be allowed to complete the single 23 day suspension by February 5, 1996.

Within five days from receipt of this Consent Order by Respondent or his counsel, Respondent shall supply a copy of this Consent Order to ( confirming by carbon copy to the State Board of Physical Therapy Examiners):

(1) the Chief Executive Officer or Administrator of each hospital or other institution where Respondent holds or maintains staff privileges of any kind, and
(2) any person with whom Respondent is associated in practice, including other health care providers, and
(3) any person or entity by whom Respondent is employed as a physical therapist in this State, and
(4) the Chief Executive Officer of any and all professional associations in which Respondent currently holds membership.


AND IT IS SO ORDERED.
