ORDERS:

CONSENT ORDER OF DISMISSAL

IN RE: Suspension of Petitioner's Paramedic Certification

The parties have settled the above-referenced contested case. Without a finding of fact or law, and without an admission of liability or wrongdoing on the part or Petitioner, the parties have agreed:

1. Petitioner shall, within six months of the date of this Order, take a paramedic refresher course.

2. Petitioner shall, within one year of the date of this Order, pass the State Paramedic written examination. Petitioner shall have two opportunities to pass the written examination.

3. Petitioner shall, within one year of the date of this Order, shall take and pass the paramedic practical examination, consisting of all four (4) paramedic skill stations. Petitioner shall have two opportunities to pass the practical examination.

4. All parties agree that if Petitioner fails to complete any of the requirements set forth above in paragraphs One through Three, upon such failure, Petitioner's Paramedic Certification shall be suspended for a period of three (3) years.

THEREFORE, upon motion of the Respondent, by and with the consent of the Petitioner, this Contested Case is hereby dismissed.

IT IS SO ORDERED.
