ORDERS:
CONSENT AGREEMENT AND ORDER
This
matter is before me pursuant to a request for contested case hearing filed by
Respondent Zanders Transport Service challenging the decision of Petitioner
South Carolina Department of Health and Environmental Control revoking
Respondent’s license to operate an ambulance service. Prior to hearing, the
parties reached an agreement resolving this matter. As indicated by their
signatures below, Zanders Transport Service agrees to the revocation of its
ambulance service license. Zanders Transport Service further agrees that it
will not apply for a new ambulance service license for a period of seven (7)
years from the date of this Consent Agreement and Order. The parties request
that this contested case hearing be dismissed.
IT
IS THEREFORE ORDERED that this matter is dismissed with prejudice.
